Medinvents is a new company founded by physicians that care about cancer patients.  The mean product is a soft tissue large core biopsy device, the Spirotome, that offers large core biopsies without causing stressful intervention emotions to the patient.  The system is being automized in order to facilitate the procedure even more.  In addition, cosmetic products are offered to alleviate common side effects or problematic situations secondary to cancer treatments.

THE SPIROTOME

 

 

 

Set of 3 needles that make the Spirotome.  The set is designed to take large core (8 to 12 Gauge macro-) biopsies from virtually every soft tissue, in particular from the breast.  The material is non-magnetic and can be used clinically, with ultrasound, CT and MRI.  The lengths of the set can be up to 10, 8 and 6 cm with 3 dimensions: 3 (minor), 4 (standard) and 5 (jumbo) millimetre maximal diameter.  The sets are available as reusable and single use.  The single use versions have a valve mechanism to create reversible vacuum in the helix during penetration, cutting and harvesting.

 

The Spirotome is manually usable.  There is no shooting, avoiding vital or impenetrable organs to be accidentally touched as bones, longs, vascular vessels etc.  The procedure can be stopped at every moment by unscrewing the Spirotome from the tissues.  The Spirotome can be used for every soft tissue biopsy (breast, gland….) no matter what the dept of the diseased area is.  It can be used for superficial tumours or deeper situated tumours, tumours near to the thorax, to the bone etc.    The procedure can be done virtually everywhere: on a simple operating table, bed, private praxis etc. without or with little assistance.  The biopsy takes only 10 minutes, local anesthesia included.  The biopsy specimen is large compared to the diameter of the needle set.  2 to 3 mm specimens are obtained with a needle with a maximum diameter of 4 mm.  The Jumbo can produce specimen up to 4 mm diameter.  The reusable Spirotome can be sterilized up to 10 times.  No accessories are needed.  The tissue is not damaged because it is protected through the helix before, during and after cutting.  The helix has high visibility on ultrasound and other imaging techniques.  Hereby, the examiner can see under ultrasound how and to what extent the helix penetrates the diseased area.  It brings absolute medico-legal proof that the diseased area indeed is harvested.  Usually one insertion of the needle is necessary for a representative biopsy so that seeding of tumour cells can be overcome.  .The skin entering part is a trocar but the tumour entering part is a very tiny point of the helix.  This is an advantage for example, a fibroadenoma can first be 'hooked up' before the cutting action is exerted.  The system is proofed to be ideal for stereotactic tables and MRI imaging.  For the stereotactic tables, the Spirotome was elected as most performing for regular use at the tables.  Since the Spirotome is the only system with no magnetic characteristics its use in MRI imaging is obvious.  The breast array coil is adapted for the Spirotome.

The Spirotome system can be ideally adapted for use for stereotactic table.  The needles are somewhat longer to be positioned on the interfaces of the table.  Interfaces are obtainable on request.

The reusable Spirotome needles for punch biopsies are an excellent alternative to the combination of disposable punch needles that have to be used in conjunction with reusable scissors and pincers or forceps.  The Punch Spirotome is all in one.  The skin is hooked up with the helix, the cutting cannula is rotated over the helix thereby cutting the cylinder.  The helix is retracted and the cylinder can be found in the helix wherefrom it is easily removed by the releasing element.

The system works fine for removing tattoo’s from radiotherapy that cause an esthetic problem for patients.

 

The Standard set has a punch diameter of 4 mm, the Jumbo of 5 mm.  The releasing elements are accordingly sized.

The reusable Spirotome needles for cervix biopsies are an excellent alternative to the combination of a reusable Pozzi cervix hooking forceps and a superficial cutting device.  The Cervix Spirotome is all in one.  The cervix is hooked up with the helix and penetrate as deep as the biopsy is needed.  The cutting cannula is rotated over the helix thereby cutting the tissue cylinder.  The helix is retracted and the specimen cylinder can be found in the helix wherefrom it is easily removed by the releasing element.

 

The Standard set has a punch diameter of 4 mm, the Jumbo of 5 mm.  The releasing elements are accordingly sized.

The releasing element is used for fast and safe removal of the harvested specimen from the helix.  The filled helix is positioned on the teeth of the releasing element.  The teeth don’t penetrate the tissue specimen but hold the tissue cylinder while the helix is rotated counter-clockwise.  The freed tissue remains on the teeth after complete removal.  Without further action the tissue can be dropped in the fixator for pathology.

Safety Issues
The Spirotome system is a safe large core biopsy system provided essential safety rules are considered.  Among them these are the most important:
  • The system should only be operated by experienced physicians after a training course.
  • Never use the Spirotome for other purposes than soft tissue biopsies.
  • Apply properly sterilized materials only.
  • Use an appropriate local anaesthetic throughout the biopsy tractus.
  • The cross skin incision (scalpel) should be larger than the diameter of the cutting cannula.
  • The penetration of the Spirotome into the human body should be closely followed by palpation and, when possible, preferentially by ultrasound or other imaging techniques.
  • Use the imaging techniques for localizing dangerous (e.g. blood vessels), labile or bony structures.
  • The receiving needle should only be turned - never pull or push the needle since this will deform the helix !
  • The tissue sample should be immersed immediately in the proper fixator that has been delivered by the pathology department of the institute.
  • Label the tissue container properly to avoid mismatching of the samples
